Consensus Update on Definition and Diagnosis for Sarcopenia

Abstract
Sarcopenia is defined as the loss of skeletal muscle mass and strength with increased age. This finding results in weakness, limited mobility, and increased susceptibility to injury. The risk of disability is much higher in older persons with sarcopenia than in older persons with healthy muscle. Prevention, treatment, and rehabilitation of these conditions have become a major concern in health care today. For the clinical application of sarcopenia in medical practice, its recognition as a disease and consensus on diagnostic criteria have highest priority, with emphasis on basic research and accumulation of clinical evidence. Since European Working Group on Sarcopenia in Older People (EWGSOP) suggested the diagnostic criteria including walking speed and grip strength in addition to the muscle mass in 2010, the diagnostic criteria and algorism have been suggested differently between the groups such as EWGSOP and Asian Working Group for Sarcopenia (AWGS). The diagnosis process of sarcopenia is complicated, the cutoff values are variable, and the measurement of muscle mass and quality is not accurate. Although sarcopenia is a geriatric giant that attracts attention from health and medical care, that has been a major drawback to create the clinical standard for diagnosis and treatment. Since 2018, a lot of ground-breaking reports have been released on the consensus and clinical guideline for diagnosis and management of sarcopenia. Revised European consensus and revised Asian consensus on definition and diagnosis of sarcopenia are the mainstream. Since the guideline of AWGS were released in 2013, AWGS has resumed active discussions on revisions in December 2018. In the review, I will illustrate the development, discussion and consensus process for the revision of the Asian Guidelines and introduce new algorithms and cutoff values for diagnosing and evaluating sarcopenia. I would like to discuss the differences from the European guidelines and suggest future tasks.
